Playing all this Organ Trail gave me an idea: Arkham Trail, which is Oregon Trail in the Cthulhu Mythos. You'd play as a party of escaped asylum patients (who were imprisoned for cult activity) in Providence, where you stole the van of the asylum and are on a journey to Arkham because you heard there were cult gatherings there. Along the way you'd run into familiar locales like Innsmouth and Red Hook and encounter characters like Herbert West and Randolph Carter.
Random events would include nightgaunts carrying away party members, being forced to outdrive a shoggoth, being ambushed by a rival cult, and catching dysentery. There would also be a 'divinity' stat for your party, which you can increase by sacrificing people. Every time your divinity meter fills up you gain large quantities of resources or upgrades for the asylum van. At the start of the game, you'd choose which eldritch abomination to follow, which would effect what your upgrades your divinity meter grants.
While doing what would be the equivalent to the hunting minigame from the original, you could run into people (which you could kill to gain divinity), ghouls (which can take some of your food), star vampires (sap health away metroid-style) and rarely, bosses like Elder Things, Shoggoths, Hounds of Tindalos, etc.
Missions would be anything from 'finding' bodies for Herbert West, killing a Hound of Tindalos to help out a time traveller, and smuggling items into certain areas. The rewards would range from common resources, to weapons like sacrificial daggers (divinity bonus with each kill) and archaic runes (ranged, extra damaged to non-humanoid enemies), to special only-one-in-each-playthrough one-use items like regeneration serum (bring back a party member, although with only one health), Yith Mind (travel instantly to next landmark), and Twist of Fate (makes all resources much more common for a limited time).
Upon reaching Arkham, the cult leader will rank you based on the amount of divinity you'd accumulated over the course of the game. Some of the rankings would be 'Sacrifice', 'Weekend Cultist', 'Thug', 'Worthy Follower', 'Acolyte', 'Priest', 'High Priest', 'High Priest Not To Be Described', and finally, 'Eldritch Demigod'.

So I found this iOS game called 'Blood Roofs'. It's basically temple run with grotesque biological monstrosities in a victorian-era city. While the gameplay has a few issues, mostly with the annoying camera, the monster designs are very cool. They include fleshy, crocodile-headed beasts, big spider legs from some unknown monstrosity, and a giant worm-monster with loads of teeth.
