What or who makes squeeze damage? I first thought it could be bites, but according to armour analysis tear also include bites. Is squeeze damage applied, when I fall down a tree? Or are there enemies that "hug" me? "Bear hug" comes to mind...

Pretty much only bears. BEAR HUGS ARE DEATH HUGS. Even then it seems very rare.
